Prevention Assessment Team (WSCC & NHS)

The Northern Prevention Assessment Team (covering Crawley, Horsham and Mid Sussex) includes, health advisors (nurses), social care workers and support workers. The team aims to help people to stay healthy and remain independent. Normally they work with people who do not receive services from specialist health care teams or social care services. The team members can check your general state of health and wellbeing, help identify what may benefit you to retain your independence, provide assistance to access practical, emotional and social support or link you with agencies to ensure you are claiming the benefits you are entitled to.
