Wheelchair Service - West Sussex (Sussex Community NHS Trust)
The Wheelchair Service will loan a wheelchair to anyone who has a long term mobility problem and is classified as unable to, or virtually unable to walk indoors or outdoors. The wheelchair must also be required for at least 6 months.
You must be registered with a West Sussex GP and living in your own home. Referral forms will be accepted from GPs, medical consultants and other health care professionals, such as Occupational Therapists and Physiotherapists.
